How to Enroll

Join hundreds of thousands of Rhode Islanders who already count on CurrentCare and CurrentCare for Me to coordinate and improve their care.

Signing up is easy. Online enrollment takes just minutes and it is available 24 hours a day – or you can sign up at your doctor’s office or other healthcare facilities. 

There is no cost to enroll in CurrentCare and CurrentCare for Me. It is a free service provided to you by the Rhode Island healthcare community.

Enroll Online

Enrolling online is quick and easy. All you need to do is provide some basic information about yourself. CurrentCare confirms your identity by asking you a few questions that only you can answer. Then, you choose from one of three options for the level of access you want to give your providers.

That’s it, you’re done! Your healthcare team can now begin to use the power of information to coordinate around you. Enroll in CurrentCare and CurrentCare for Me now.

Enroll in Person or by Mail

To enroll in person or by mail, download CurrentCare and CurrentCare for Me Enrollment Form [PDF, 333KB] and follow the instructions below.

Be sure to complete all information and sign the form. To protect your privacy, we confirm the identity of every enrollee. To complete your enrollment in CurrentCare you must verify your identity in one of the following ways:

  1. Have your primary care doctor verify your identity and submit the form for you. If your primary care doctor doesn’t participate in CurrentCare, then you must use option 2 or 3 below.
  2. Have your signature witnessed/verified by a notary public and mail the form to:
        Rhode Island Quality Institute
        50 Holden St., Suite 300
        Providence, RI  02908
  3. Bring the completed form to RIQI for verification. Be sure to bring your government-issued photo ID and ask for a member of the Operations Department.

Add a Designee

With CurrentCare for Me Designee access, you can choose to have other individuals access your record. Learn more about designees here.

To add a designee by mail or in person, download the CurrentCare Designee Form [PDF] and follow the instructions below.

Be sure to complete all information and sign the form (note: your designee does not need to sign… they will receive a welcome email with instructions). To protect your privacy, we confirm the identity of every signature using one of the following ways:

  1. Have your primary care doctor verify your identity and submit the form for you. If your primary care doctor doesn’t participate in CurrentCare, then you must use option 2 or 3 below.
  2. Have your signature witnessed/verified by a notary public and mail the form to:

  3. Bring the completed form to RIQI for verification. Be sure to bring your government-issued photo ID and ask for a member of the Operations Department.
